refers to the act of a person’s head or upper body breaking the plane of an opening into a space not designed forcontinuous human occupancy. Because these areas—such as tanks, silos, sewers, or pits—are often poorlyventilated and difficult to exit, entering them is strictly regulated to prevent fatalities from toxic air, lack of oxygen,or physical hazards.
